Essential Duties and Responsibilities: The Transition Manager must be able to articulate program values to potential members, natural supports and external stake holders. The transition manager must know and adhere to Group Effort’s policies and procedures and philosophies, and goals. Have the technical knowledge and competence in the agency recruiting, admission policies and transition process.
Communal Duties include: Working a flexible schedule which will include occasional weekday or weekend overnight travel, serving on Group Effort committees; this includes:
• Participating in the Supervisor “on-call” rotation.
• Available by phone (24/7)
• Provide coverage for unscheduled shifts as needed
• Participate on the Incident Management Committee
• Assist with TFM training with FMP
• Act as an agency liaison.
Recruiting Duties:
• Schedule and complete FMP home studies
• Create family model provider profiles
• Schedule and complete FMP home assessments
Transition Duties:
• Respond to referrals within 24 hours.
• Coordinate Meet and Greets with agency personnel and third-party stakeholders
• Provide or schedule transportation for a member to attend meet and greets
• Create member profile in PHS
• Establish member home and office book
• Assist or complete Day of Transition process and welcome packets
Position Qualifications:
This position requires High School diploma or GED. Bachelor’s degree is preferred. Must have 1 year of experience in a community based long-term care or residential program. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

• Operation of a motor vehicle is required which includes a current, valid Tennessee driver’s license and applicable automobile insurance.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute percentages
